Qualifications

5. Ability to assess, manage & treat patients with complex multi pathologies at highly specialist level
6. Skills in goal setting & using outcome measures
7. Knowledge and skills in discharge planning and onward referral

10. Ability to produce clear written documentation of intervention & reports
11. Venepuncture and cannulation

Experience
Essential criteria
19. Experience of Elderly rehabilitation within the acute and community settings.
20. Experience of facilitation of the hospital discharge process

23. Recognition of deteriorating patient
24. Minimum of 18 months experience including an acute NHS setting

Knowledge
Essential criteria
27. Understanding of the pathologies that elderly patients commonly present with
28. Therapeutic intervention for patients with chronic, multi pathology and complex conditions
29. Understanding of NSF’s, Clinical Governance and their relevance to clinical situations
30. Good knowledge of long term conditions, Dementia, Falls, Fractures, Osteoporosis and infections commonly affecting the older person
31. Understanding of NSF’s Clinical Governance for Older People, Falls, Parkinson’s and Intermediate Care
32. Understanding of the interface between hospital, intermediate care and community services
33. A sound understanding of capacity issues and ability to recognise need for formal assessment

34. Ability to use a keyboard to access and input information from a computer based system
35. Ability to move and handle patients and equipment and work in confined spaces
36. Ability to participate in extended hours and weekend service
37. Ability to drive, hold a drivers licence valid in the UK and have access to a car for work purposes
