We are excited to offer an excellent opportunity for an experienced Occupational Therapist or Physiotherapist to join us as a Team Leader within our integrated Community Therapy Team, serving the South Lakes and Furness areas. This role provides the chance to lead a dedicated, multidisciplinary team, supporting the delivery of high-quality, person-centred care to our local communities.
As a Team Leader, you will:
Support Community Therapy and Rehabilitation Services, ensuring safe and effective care for patients.
Lead and oversee Therapy Discharge pathways, promoting smooth transitions from hospital to home.
Champion the Wellness Model, focusing on holistic, person-centred care that empowers independence and recovery.
Manage the day-to-day running of the service, working collaboratively with a team of Band 7 Clinical Leads and a fellow Joint Team Lead.
Ensure service delivery standards are met, monitoring performance and driving continuous improvement.
Work closely with senior leaders, receiving guidance and support to achieve operational and strategic goals.
Provide leadership, mentorship, and professional development opportunities for team members.
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
