Active Care Group's Care in the Home division continues to grow, providing safe, reliable, and outcome-based care to all clients through our exceptional support workers, clinicians, and management team.

Due to ongoing expansion, we are opening a new hub in Thame, Oxford, and are seeking to expand our team of Care Coordinators within the Care in the Home division. This is a fantastic opportunity to join our friendly team, supporting children, young people, and adults with conditions such as brain injury, spinal cord injury, neurological and neurodegenerative diseases, respiratory, and gastrointestinal conditions, including PEG feeding.
When you'll be working:
* 37.5 hours per week, office-based
* 9:00 am to 5:30 pm
* On-call rota, 1 week out of every 8
What you'll be doing:
* Handling inbound and outbound calls to clients and workers to check welfare and shift coverage
* Updating the Team Leader on critical team matters daily
* Ensuring compliance with company policies and regulatory standards, including safeguarding, complaints, and continuity of care
* Communicating effectively with staff at all levels, clients, and the public, especially when handling complex or sensitive information
* Contributing to innovative ideas and supporting a flexible, responsive workplace
* Delivering best practices and ensuring timely workload management
* Representing the company positively and maintaining strong relationships with stakeholders
What you'll have:
* Experience as a Care Coordinator
* Familiarity with booking and phone systems
* Conflict resolution skills
* A strong work ethic, flexibility, and a proactive approach
* Excellent customer service and communication skills
* Teamwork and independent working capabilities
* Willingness to learn and develop
* Enhanced DBS clearance (or willingness to undergo one)
