Role Summary

An exciting opportunity has arisen for a Specialist Occupational Therapist or Physiotherapist to join our Urgent Community Response (UCR) team covering Honiton, Ottery St Mary and Cranbrook. You’ll deliver rapid, person-centred assessment and intervention to help people in crisis stay safe and well at home, avoiding unnecessary hospital admissions. Working as part of a friendly, skilled multidisciplinary team, you’ll use your clinical expertise to make timely, autonomous decisions and support smooth care pathways. We’re looking for a compassionate, proactive clinician with strong assessment skills, a flexible approach, and a full driving licence (pool cars available). An exciting opportunity has arisen within our Urgent Community Response (UCR) service following the promotion of the existing post holder. We’re looking for a motivated and compassionate Specialist Occupational Therapist or Physiotherapist to join our highly regarded team, covering the beautiful East Devon localities of Honiton, Ottery St Mary and Cranbrook.

If you’re passionate about providing person-centred, responsive care that keeps people safe and well in their own homes, as well as facilitating safe and efficient hospital discharges, and you thrive in a fast-paced, supportive multidisciplinary team – we’d love to hear from you.

What you’ll do

1. Provide rapid, holistic assessments in patients’ own homes and community settings.

2. Make timely, autonomous decisions to manage risk and promote safe independence.

3. Implement evidence-based interventions to support recovery, safety, and wellbeing.

4. Work collaboratively with community, acute, and social care partners to deliver seamless, integrated care.

5. Support clinical triage and prioritisation of UCR referrals on a rotational basis.

6. Contribute to service development, share knowledge and skills with all staff and mentor colleagues as part of our supportive learning culture.

What we’re looking for

7. A registered Occupational Therapist or Physiotherapist with HCPC registration.

8. Strong assessment, decision-making, and risk management skills.

9. A flexible, proactive approach and confidence working both independently and as part of a team.

10. A valid driving licence and access to a car (pool cars also available).

11. A genuine passion for providing high-quality, person-centred community care.

12. Experience of working with the acutely unwell or deteriorating patient, knowledge of community services and/or a previous role in a rehabilitation-focussed team.
