The team is based within various locations across Angus. Working within a team you will provide care and support to patients and support to their carers within a clinic setting. Angus Health and Social Care Partnership is in the implementation phase of Transforming Primary Care Services and part of this new work is to develop and embed a new model to provide treatment room care for patients in Angus. You will have a desire to work in the community, be well organised and have excellent communication skills, as well as experience of working within a health or social care setting. Ability and willingness to travel throughout the area of service delivery is essential for this post to provide patient care.
