Job Overview
As a Senior Care Coordination Assistant, you will play a vital role in ensuring the delivery of high-quality care to our clients. You will be responsible for coordinating staff rotas and care plans for people receiving social care and support. This position requires excellent communication skills, a strong understanding of care needs, and the ability to work collaboratively with various stakeholders to enhance the well-being of our clients.
Duties
* Support and guide care and support staff in delivering high-quality care and support, including conducting spot checks.
* Attending service reviews with professionals and service users or their relatives
* Support managers in monitoring staff performance and activities.
* Coordinate staff rotas and allocate carer and support worker duties based on the needs of clients and service users.
* Conduct assessments to evaluate client needs and preferences, ensuring that all aspects of their care are addressed.
* Develop and implement personalised care plans tailored to the individual needs of clients.
* Monitor the effectiveness of care plans and adjust as necessary based on feedback and changing needs.
* Answer incoming phone calls, deal with queries promptly, and support junior staff who assist with answering calls.
* Support with audits and other Quality Assurance processes and KPIs to ensure high-quality care delivery and compliance with regulatory standards and best practices within the care sector.
* Coordinate with managers, the care coordination team, support staff, and other external professionals and families to ensure seamless delivery of care services.
* Maintain accurate records and documentation related to client care and progress.
* Facilitate communication between clients, families, and healthcare providers to ensure clarity and understanding of care requirements.
* Renumerated On call support (out of hours / weekend).

Skills
* UK Full Driving Licence
* Direct care experience is essential.
* Proficient in English with excellent verbal and written communication skills.
* Strong IT skills for maintaining records, documentation, and communication with stakeholders.
* A valid driving licence and ability to travel between locations as required.
* Knowledge of care service operations is highly desirable.
* Ability to develop comprehensive care plans that meet individual client needs.
* Strong organisational skills with attention to detail to ensure all aspects of client care are managed effectively.
* Excellent interpersonal skills to build rapport with clients, families, and team members.
