Offer description

This role covers the North locality of Wiltshire, and you will be expected to travel across the area. Therefore, having a driving licence and access to a vehicle is essential.

* Participate in assessment and care planning of patient care in conjunction with the Team Manager and support of specialist team members.
* Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan. Escalate any deviation from the care plan or change in condition to the Team Manager/GP accordingly.
* Communicate effectively with patients, carers, and all members of the multidisciplinary team.
* Prioritise daily work in line with clinical/patient need.
* Maintain accurate and legible patient records in line with professional and organisational requirements, including electronic and paper records.
* Implement and work to ensure evidence-based care and best practice.
* Actively participate and contribute to team meetings and daily handovers.
* Initiate health promotion activities with individuals, patients, and carers.
* Be involved in the development and implementation of community nursing service developments and initiatives.
* Participate in clinical audits and implement changes based on findings and recommendations.
* Participate in risk assessment and promote safety and well-being at all times.
* Ensure all complaints, incidents, accidents, and clinical risks are identified and reported according to organisational policies.
* Stay up-to-date with professional requirements, healthcare developments, clinical evidence, and policies.
* Complete all mandatory training within the required timeframe.
* Attend and participate in regular clinical supervision, peer reviews, and annual appraisals.
* Undertake mentorship training and facilitate teaching and learning for students and staff.
* Be accountable for own professional actions in line with the NMC code and organisational policies.
* Monitor and order stock and equipment in line with budget and lean principles.
* Ensure the security, care, and maintenance of equipment, maintaining infection control and safety standards.

Front line NHS community care experience, up-to-date clinical professional development, excellent clinical and communication skills, ability to work under pressure, motivated, flexible, and hardworking individual, registered with the NMC, evidence of CPD, mentorship/teaching qualification (within first 2 years), educated to diploma level, and ability to work within a team and independently are required.

Knowledge of recent developments in health and social care, evidence-based practice, research, ability to operate community equipment, assess needs, and pass urgent information is essential. Flexibility with working hours across a seven-day week, ability to prioritize visits, organize workload, and input data electronically are also required.

Desirable qualifications include degree level education. Other requirements include a current driving license and access to a car during working hours. Confident IT skills, including Microsoft Office, are necessary.
