Overview
The post holder will predominantly work independently in the community setting with adults referred to the team who present with a range of neurological conditions. You will manage a caseload of non-complex patients, completing assessments independently, setting short term therapy goals and treatment plans, completing timely reviews and discharging patients from the service. The nominal base will be at Sedgefield Hospital; the post may need to cover a county-wide area. You will be expected to travel across the whole South/DDES Durham locality as required by the service. You will need to complete documentation at the point of care and use a lone worker system in compliance with Health and Safety. The post holder will work within a multidisciplinary team for the management and discharge of patients under your care.
Responsibilities

Person Specification / Qualifications

* Excellent teamwork skills with a commitment to providing seamless, holistic, patient-centred care within a multidisciplinary team.
* From 9 April 2025, Healthcare Assistants must have a minimum of two years of NHS experience as an HCA for sponsorship consideration (sponsorship only if criteria are met and visa expiry within 3 months).
* Must be able to produce all certificates stated as essential in the person specification to complete pre-employment checks.
