We are looking to recruit an enthusiastic and motivated person to a part time permanent Assistant Practitioner post within the friendly, dynamic and supportive Adult Speech and Language Therapy team. We work across Nottingham city and are currently based at The Meadows Health Centre.
You will work as an integral member of the Adult Speech and Language therapy team, providing face-to-face rehabilitation to clients with swallowing and communication difficulties in community settings, such as their own home, care homes or clinic settings. A key aspect of the role is to deliver communication group therapy and training to others, including care home staff, with supervision from qualified clinicians. You will also provide support to the speech and language therapists by preparing therapy resources, such as communication picture aids, and collating medical information from medical notes, patients and carers to support with the triage process. Having a specialised role in working with people with swallowing disorders (dysphagia), such as carrying out screening assessments and reviewing swallow care-plans, you will be given the necessary support to complete our CityCare competency framework for Assistant Practitioners, with team specific competencies in communication and swallowing disorders. You will also have regular opportunities for continuing professional development through attending our regular in-service training as well as relevant Clinical Excellence Network meetings
Main duties of the job
The Speech and Language Therapy team work across Nottingham, supporting a wide variety of adult clients in their own homes, community settings including clinics, care and nursing homes. We offer virtual consultations and interventions. We work closely with other community health and social care teams, and the voluntary sector. Our clients present with a wide range of aetiologies including stroke, end of life, dementia, neurological, mental health and respiratory conditions.
Your responsibilities will include working autonomously to meet individual patient needs. You will have the ability to plan ongoing clinical interventions e.g. home visits, organising appointments, clinics and assessments. You will also be an excellent communicator with patients, carers and within the team. You will be quick to report any concerns you have about patient care and regularly raise any questions or ideas relating to the service with the rest of the team. You will have excellent rapport building skills with patients and their carers, as well as conflict resolution and negotiation skills.
You will have knowledge and experience of patient care in a health and/or social care setting and will be educated to Foundation degree or equivalent, in a health related discipline or have evidence of successful completion of all Citycare Band 4 AP competencies.
The post consists of 30 hours per week
A driving licence/ vehicle is essential.
