Leeds Outreach
We offer care packages for a diverse range of individuals with different care and support needs, which range from full support, forensic, challenging behaviours whilst being able to risk assess situations. There is also support helping with budgeting and shopping.
For the people we support you would need to be very active to support with social activities as they like to go Swimming, to the gym and go to the Rugby games or concerts on occasions so this may include late evenings.
In our Leeds Outreach you will be working within the community and within peoples own homes and day centre support, supporting with daily finances, meal preparation and personal care and every day is different, as each individual is different, this is why a person centred approach is essential.
Across all services, the individuals we support have autism, learning disabilities and at times can display behaviours of concern, therefore you must have the ability to follow detailed support plans in order to ensure a consistent approach.
