Person specification
Experience
Essential criteria
1. Experience of working in a hospital setting or relevant community service
2. Understanding of complex discharge planning in care of adults and vulnerable individuals.
3. Experience of supporting patients’ specialised care needs to enable informed decision making
Desirable criteria
4. Previous experience of working with patients requiring CHC and Social care involvement.
Qualifications

5. Good standard of education
Desirable criteria
6. NVQ level 3: health / social care management or equivalent
Technical Skills Competencies
Essential criteria
7. Assessment, planning and organisational skills
8. Ability to progress plans for discharge with internal / external agencies and family members
9. Able to intergrate and work within the MDT
10. Time Management Skills
11. Use of IT including Windows, Word, Excel, Powerpoint, ECAMIS, ENA,
12. Good verbal communication skills both face to face and over the telephone.
Desirable criteria
13. Use of EPR, Mosaiq, eforms systems
Knowledge
Essential criteria
14. Community referral processes
15. Knowledge of the complex discharge planning process.
16. Understanding of assessing and problem solving issues related to discharge planning
17. Understanding of multidisciplinary assessment and treatment for adults/ vulnerable individuals within acute care and community settings

18. Understanding of the new Care Act, NSF’s, CHC Framework, Delayed Discharged Act and Reimbursement
