*Bank Band 5/6 Community Mental Health Nurse *
We are seeking Band 5 and Band 6 Mental Health (RMN) Community Nurses who have a keen interest in working in the Community Mental Health Teams in Tower Hamlets. You will have the opportunity to develop your community nursing skills and support the teams to meet key targets and provide excellent care for service users and keep them well in the community.
You will be part of a pool of bank nurses who we will call upon when we need bank nurse cover. You will be provided with an induction to each of the 5 community teams so you are ready to support any of the teams when they need someone

Main duties of the job
1. To be an active member of the multi-disciplinary team providing assessment, care and
2. support to adult presenting with mental health issues within the neighbourhood Team
3. To conduct comprehensive and high-quality holistic Health and Social Care (Care Act 2014) assessments including safeguarding when required.
4. To act as a care co-ordinator for a caseload of service users and develop, implement and review individualised recovery focused care and support plans and commissioned packages of care.
5. Use a broad range of nursing knowledge, interventions and skills, to prevent, where possible and avoid hospital admission
6. To conduct carers assessments and offer support and advice to carers and relatives.
7. To promote the needs of service users and offer and provide training and education accordingly.
8. To operate as a Duty Worker on rotation across the team in line with the Mental Health Transformation Project, responding to emergencies, processing referrals and presenting at referrals meetings of your assessment.
9. To support the seamless running of the depot clinic
10. Develop good communication links with patients and carers and joint working with other services across professional and organisational boundaries in order to provide a patient-centred, holistic and seamless service

Essential criteria
11. •A graduate mental health nurse who is pending their PIN or a Registered Mental Health Nurse or a AHP Registered professional .
12. •Willingness to undertake further training relevant to the role.
Experience
Essential criteria
13. Experience of mental health nursing either in inpatient or community setting.
Knowledge and Skills
Essential criteria
14. •Knowledge of the Mental Health Act (1983), Mental Capacity At (2005) & the Care Act (2014. •Understanding of the principles of the recovery model and Care Programme Approach
15. •Understanding of the principles of the recovery model and Care Programme Approach
16. •Up to date knowledge of the current approaches to mental health nursing.
17. •Up to date knowledge of the issues that impact individuals and carers affected by mental health and / or cognitive impairment (where applicable
18. •Good verbal and written communication skills.
19. •Ability to assess, plan, implement and evaluate care using evidence based approaches
20. •Ability to prioritise workload.
