Would you like to help bridge the gap between healthcare and the community by supporting individuals to take control of their wellbeing and connect with the support that matters?
Ruils is a charity supporting disabled children and adults, to live independently, be part of their community and to live life to the full. They provide information, advice, practical support, befriending and accessible activities to our clients and families.
They are proud to be delivering Social Prescribing in partnership with local Primary Care Networks in Richmond. This innovative, person-centred programme forms part of the NHS Long-Term Plan, enabling health professionals to refer individuals to Link Workers who help connect them with community-based services and support.
This role focuses on empowering people to take greater control over their lives and wellbeing. Many clients will be facing complex social, emotional or practical challenges, so we are seeking someone with excellent listening skills, empathy, and resilience.
You don’t need to have done this exact job before. You might be working in health or social care, education, the voluntary sector – or another role where you’ve supported people to overcome barriers and improve their wellbeing.
You’ll be:
* managing a caseload and providing tailored one-to-one support to clients
* co-producing action plans and connecting individuals to community resources
* building strong working relationships with local GPs and partner organisations
* maintaining accurate records and contributing to service development.
Amongst the skills you need are strong interpersonal and communication skills, an organised and proactive approach, a genuine commitment to person-centred support and a knowledge of the local area and services (desirable but not essential)
