We have an exciting opportunity fora Band 6 Occupational Therapist to join our Reablement Team based in Bath. This is a very rewarding role within the Reablement Service which is a short-term service which supports people in their own home or in Residential or Nursing Homes aiming to maximise and maintain independence through the provision of rehabilitation and support.
You will join a supportive and multi-disciplinary team consisting of Reablement Therapy Workers, Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners, Care Co-Ordinators and Administrative support. The team also works closely with local GPs and other community services such as District Nurses and Social Workers. This role will involve travel around Bath and North East Somerset area so a Full UK Driving License and access to a vehicle for work purposes is essential. This is a full time position based at our St Martins Hospital, Bath.
Main duties of the job
As a Reablement Occupational Therapist you will:
Carry out therapeutic assessment, diagnosis and treatment with patients using person centred goals.
Provide an evidence-based Occupational therapy service of a high standard, to patients in a community setting by undertaking comprehensive assessments, reassessments, care planning, rehabilitation and therapeutic care and support.
Hold responsibility for own case load working with rehabilitation therapy workers to deliver therapeutic programmes
Provide effective supervision to junior Occupational Therapists, Therapy Assistant Practioners and Reablement therapy workers.

* You must hold a Degree in Occupational Therapy
