CHOC PCN are looking for an enthusiastic and motivated Pharmacy Technician who shares our values and is committed to providing excellent care, to join our growing team. You will support GP practices and work closely with our team of Clinical Pharmacists, acting within professional boundaries to support a multi-disciplinary team within general practice and a Primary Care Network.
Job Summary
The purpose of the role is to lead improvements to maximise safe, cost effective best practice in prescribing to improve patient care. You will help patients get the best from their medicines by switching medications to agreed protocols, improving repeat prescribing processes, promoting digitalisation, minimising clinical risk, and reducing wasted medicines. You will help develop medicines optimisation understanding across practice teams and promote good practice in line with therapeutic developments, including implementing NICE guidelines and medicines optimisation initiatives.
Main duties and responsibilities
* Undertake patient facing and supporting roles to ensure effective medicines use
* Carry out medicines optimisation tasks including medicine administration, reconciliation, and actioning hospital-initiated medication changes
* Work with patients to ensure they use their medicines effectively
* Support medication reviews and reconciliation for care home patients and synchronise medicines for patient transfers between care settings, liaising with local pharmacists
* Provide public health and social care advice and information
* Lead in shared care protocols and liaise with specialist pharmacists for complex patients
* Support antimicrobial stewardship initiatives
* Assist in medicines optimisation, management incentive schemes and patient safety audits
* Support the Pharmacy team with Structured Medication Reviews
* Work with the PCN multi-disciplinary team to optimise medicines use and reduce waste, including ordering and return processes
* Provide leadership for medicines optimisation systems, including Electronic Prescriptions and safe repeat prescribing, with monitoring of high-risk medicines
* Provide training on the legal, safe and secure handling of medicines
Qualifications
* Qualified, registered Pharmacy Technician with experience in primary care, community, or hospital pharmacy
* Registered with the General Pharmaceutical Council (GPhC)
* Meet GPhC criteria to register as a Pharmacy Technician
* Undertake the CPPE Primary Care Pharmacy Education Pathway (PCPEP) as required by the PCN ARRS
* Responsible for own development, CPD and ongoing professional learning

* Salary: £29,970 a year, starting from £29,970 + NHS Pension
* Contract: Permanent
