Role Overview
This is an exciting opportunity to join the Sefton community learning disabilities team as a qualified Nurse Associate. The successful candidate will work under the supervision of a Band 6/7 nurse, providing person‑centred care and clinical assessment for the physical and mental health needs of clients with learning disabilities.
Responsibilities
* Provide physical health assessments and health promotion advice in line with national guidance.
* Signpost and refer clients to appropriate services to meet identified needs.
* Collaborate with the multidisciplinary team to deliver enhanced support and monitor the impact of interventions.
* Assist in the ongoing assessment, planning, management and evaluation of care.
* Support individuals in all aspects of care, including daily living, promoting health and independence.
* Administer medicines safely in line with Trust SOPs.
* Maintain privacy, dignity and safety of individuals at all times.
* Recognise and act on changing priorities and safeguard concerns.
* Assist in risk assessment and management across the care environment.
* Implement actions to meet physical, emotional, psychological, social, cultural and spiritual needs.
* Deliver complex care as prescribed by the registered nurse.
* Co‑operate with other providers and referral systems to meet client needs.
* Engage in reflective practice and manage own professional development.
* Apply principles of consent and mental capacity law when performing nursing and care procedures.
Requirements
Qualified Nurse Associate (or equivalent Nursing Assistant Practitioner). Supervised by a Band 6 or 7 nurse. Demonstrated clinical and communication skills, team working, and a commitment to person‑centred care.
