We are looking for an enthusiastic and motivated individual to join and work alongside our very friendly team of practice nurses, GPs and Admin staff. We are offering 30 hours to cover Tuesdays, Wednesdays and Fridays and we can accommodate hours to suit.
Main duties of the job
Chronic disease management, child and adult immunisations, cytology. Work closely with the Practice nurses and nurse clinician.
Job responsibilities
* Provide a wide range of clinical nursing care skills within GP Surgery.
* Undertake chronic disease management for the practice population.
* Undertake audit.
* Undertake triage if required by the Practice.
* Promote optimum health and well‑being to the GP practice population.
* Assess, plan, implement and evaluate appropriate treatment/care.
* Undertake screening procedures e.g. cervical cytology, health checks etc.
* Maintain accurate record keeping using the IT system.
* Actively participate in Practice and multi‑disciplinary meetings.
* Maintain effective liaison with other professionals and agencies in aspects of patient care.
* In‑depth knowledge of the legal, ethical, moral and accountability issues relating to Practice Nursing.
* Maintain a professional portfolio that demonstrates evidence of ongoing professional development.
* Ensure own continued professional development.
* Access clinical supervision.
* Report and manage sickness and other absences to the Practice Manager.
* Provide timely information to the Practice Manager regarding annual leave and study leave.
Qualifications and experience
* Registered Nurse (minimum qualification).
* Experience and/or Practice Nurse qualification.
* Wide range of clinical skills.
* Experience of chronic disease management in at least two areas.
* IT skills.
* Multi‑agency working.
* Team worker.
* Good interpersonal skills.
* Research and audit skills.
* Trained in cytology screening, family planning and immunisation and vaccination.
* Experience in triage.
