PCN Pharmacy Technician -Clissold Park PCN

Clissold Park PCN is seeking a Pharmacy Technician to work under thesupervision of the clinical Lead and to ensure the safe, accurate and timelysupply of prescribed medication to patients. In addition, they will providetechnical and administrative support to the Clinical Pharmacists and Cliniciansas well as support practice primary care teams with queries around medicationfrom patients; including face to face and telephone support to patients,primary and community staff.
Main duties of the job
Clissold Park PCNis looking for a motivated individual who is flexible to respond to the developingnature of the role and the needs and priorities of individuals requiringsupport. Under the direction of the lead Clinical Pharmacist/Supervisor, weexpect that you will be able to work independently and manage your ownworkload. You will need excellent communication skills to be able to liaisewith patients, primary care staff, community pharmacists and other partners.
You will be working across the practices in the PCN and no twodays will be the same, allowing for a varied and fulfilling role. You willbecome part of a PCN Pharmacy network team, with full support from both thenetwork and the City & Hackney CCG. Training and development support isoffered.
About us
Groups of GP Practices in City and Hackneyhave recently come together to form 8 Primary Care Networks (PCNS) each coveringa total population of between 30,000-56,000.The purpose of these PCNs is to enable GP practices to work together ina collaborative way to develop and deliver network-based services that respondto the needs of the local population.
Job responsibilities
To support the PCN Clinical Pharmacists in the delivery of clinical pharmacy services;
To perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist;
To assist with medical safety monitoring system e.g. high-risk drugs;
To process repeat medication requests, including the conversion of acute requests to repeat medicines and changing quantities of medicines;
To issue prescriptions in line with agreed clinical protocols and within your competency and directing queries to an appropriate clinician;
To liaise with clinicians and administrative staff to resolve medicine-related queries;
To provide advice and guidance to patients regarding medication optimisation and lifestyle e.g. inhaler techniques;
To provide medication review services to patients via clinics in practices, domiciliary visits and in residential and nursing homes and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care;
To support practices in the achievement of prescribing Quality and Outcomes Framework targets, and other medication management targets;
To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken;
Liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care;
To monitor practice prescribing and adherence to practice, local and national prescribing policies;
To assist with the development and review of medicine audits;
To undertake specific work/prescribing initiatives under the support of the PCN Clinical Pharmacist;
Conduct medication reconciliation following discharge from hospital, clinic visits and other transfers of care e.g., new patients, care home residents), to ensure that medication changes are safely implemented in a timely fashion with the support of the pharmacist;
To promote cost effective, safe, evidence-based prescribing in accordance with local formulary, medication optimisation strategy and national guidance.
To attend local meetings seeking to improve access to medicines or repeat prescribing processes.
To establish positive working relationships and effective communication with network practices, integrated care health and care teams, and a range of local voluntary sector and community organisations;
To contribute to the development of the service, e.g., developing service pathways, paperwork and sharing good practice across network practices.
To attend and participate in practice clinical meetings and other multi-disciplinary meetings where necessary.

Person Specification
Qualifications
* BTEC/NVQ level 3 or equivalent in pharmacy services
* Enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)
Experience
* 2 year experience in a hospital, community or primary care pharmacy
* Primary care experience
