Role Summary
The post holder will be part of the Dementia Outreach Team at Airth GP Practice. This is a 37‑hour per week position that involves delivering intensive interventions to people with a confirmed diagnosis of dementia and their carers during crisis or when at risk of entering a crisis situation.
Key Responsibilities
* Deliver intensive interventions to people with confirmed dementia and their carers during crisis or at risk of crisis.
* Collaborate with health and social care professionals in primary, secondary and third‑sector community settings to meet the mental health needs of people living with dementia and promote patient‑centred care across the Forth Valley area.
* Travel throughout the service area to provide outreach support.
Qualifications & Experience
* Registered mental health nurse educated to degree level.
* Experience working within an older adult mental health setting.
* Sound knowledge of dementia and associated conditions.
* Excellent communication and time‑management skills with the ability to work autonomously.
* Post‑graduate qualifications relevant to this area or a willingness to obtain them is desirable.
* Willingness to travel throughout the area of service delivery.
Other Requirements
The successful candidate must be a member of the Protecting Vulnerable Groups (PVG) Scheme.
Working Hours & Pay
Currently the role is 37 hours per week. As from 1 April 2026 the full‑time hours will be reduced to 36 hours per week with a proportionate increase in hourly rate to maintain pay. Part‑time hours will be reduced pro‑rata.
