The primary purpose of this role is to deliver advanced-level patient care, including assessment, examination, diagnosis, and treatment, with patients at the centre of engagement, consultation, and shared decision-making. The post holder practises autonomously within their defined clinical specialty and scope of practice, as outlined in the individual role description. Advanced practice is characterised by high levels of autonomy, complex clinical decision-making, and accountability, often in situations of uncertainty and risk. It is underpinned by a Level 7 qualification, such as a Postgraduate Diploma or Master’s degree in Advanced Clinical Practice, encompassing the four pillars of practice: clinical practice, leadership and management, education, and research or quality improvement. Advanced practitioners manage care in partnership with individuals, families, and carers, applying critical analysis and problem-solving across diverse settings to develop innovative, shared solutions that enhance patient experience and outcomes. Recognition of prior learning and experience may be considered where evidence demonstrates equivalence across the four pillars.

Main duties of the job
The role involves delivering advanced clinical practice within a defined scope, in line with professional codes, national legislation and Trust policies, with full accountability for autonomous decision-making. The practitioner demonstrates expert clinical reasoning, advanced assessment and diagnostic skills, and effectively manages complex and uncertain clinical situations. Responsibilities include prescribing, ordering and interpreting investigations, admitting and discharging patients, making referrals, and managing clinical risk. The practitioner acts as a source of expert clinical opinion, role models high-quality, person-centred care, and communicates complex information sensitively and effectively across diverse populations. Collaborative working with multi-professional and multi-agency teams is central to ensuring safe, effective and coordinated care.

Detailed job description and main responsibilities
The role operates across both the Cardiac and Thoracic Directorates, providing flexible cross-cover in response to service and operational needs. The post holder leads Advanced Nurse Practitioner (ANP) led ward rounds and post-operative outpatient clinics, including specialist services such as drain clinics and Coronary Artery Bypass Graft (CABG) follow-up, to ensure safe, effective and high-quality patient care while supporting the ongoing development of the cardiothoracic practitioner role. Responsibilities include obtaining consent within scope of practice in line with Trust policy and legal frameworks, and completing appropriate training to authorise blood products.
The practitioner is required to develop and maintain advanced clinical skills through education, training and supervision, including venous and arterial access, ECG interpretation, epicardial pacing, thoracic and vascular ultrasound, echocardiography, chest drain insertion, and other specialist procedures. The role involves working a rota covering weekdays, weekends and bank holidays, with flexibility to cross-cover during periods of increased demand and readiness to undertake night shifts as part of future 24/7 service development. Ongoing professional assurance is maintained through annual practice reviews, and the post holder works collaboratively with the Lead ANP to investigate complaints and incidents, implement action plans, and embed learning to improve patient care.

1. Post Graduate Diploma (PgDip) or Masters (MSc) qualification in Advanced Clinical Practice or HEE accepted equivalence.

Previous Experience
Essential criteria
13. Evidence of research, education leadership, Quality Improvement, stakeholder engagement and influencing the development and improvement of practice, patient care, patient safety and service delivery.
14. Evidence of broad clinical experience relevant to the post, including specialist skills
15. A proven track record of staff management at a senior level.
16. Proven evidence of delivering change management, leadership skills and autonomous practice.
17. Experience of teaching patients and others including health promotion, prevention and orientation
18. Experience of working autonomously at an advanced level within the specialist area delivering effective patient focused care.
19. Evidence of providing education; teaching, mentorship, supervision and assessment of clinical practice of staff.
