The Intensive Support Service has been developed to provide a bespoke supported living services in the Hyde Tameside area of Greater Manchester to adults over the age of 18 with learning disabilities, autism and mental health needs and complex needs. The adults accepted into the service have been identified through the Transforming Care Agenda.
The service operates 24/7 using a trauma informed and case management model of care to provide support and opportunities for increased independence in the community for three adults. We have developed the new and exciting role of Service Manager to provide operational leadership of the service in a dynamic and sensitive way.
Staff members recruited will initially be required to work dynamically between the Registered Office, service, and current care provision, to ensure service users experience a well-planned transition into their new home.
