CARE Team (Complex, Assessment, Risk, Empowerment)
Be the advocate for independence. Deliver holistic, strengths-based support to adults in the community. You\u2019ll assess complex needs, manage risk, and empower individuals to live safely and well. Build strong local networks and become a trusted presence in people\u2019s lives.
* Advanced assessment and safeguarding skills
* Experience in risk management and empowerment strategies
* Court of protection experience
* A deep sense of purpose through long-term community impact

Hospital Discharge Team
Be the bridge between hospital and home. Support timely, safe, and person-centred discharges from hospital. Collaborate with health professionals, families, and community services to create tailored plans that promote recovery and independence.
* Experience of working alongside allied professionals
* Deep understanding of discharge planning and integrated care
* Skills in multi-disciplinary teamwork and complex decision-making
* The satisfaction of helping people return home safely and confidently
* Developing proportionate and timely assessment skills
Let\u2019s Talk Team
Be the first voice of support. Work with a varied team of responders, advisors, social workers, occupational therapists and community partners to prevent, reduce, and delay care needs. You\u2019ll provide early interventions, promote independence, and respond to urgent situations with compassion and clarity.
* Expertise in early help and preventative care
* Confidence in crisis response and solution-focused conversations
* A chance to shape outcomes before needs escalate
* Experience of short-term intervention and case management
* Developing skills in positive risk taking and balancing the right to autonomy
Learning Disability Team
Empower individuals with learning disabilities to live fulfilling, independent lives. You will work collaboratively to identify outcomes, develop support plans and promote inclusion and wellbeing.
