Job Summary
The post holder will undertake wound assessments and treatment for patients with both simple and complex wounds. Types of referrals may include: post-operative wounds (including suture and staple removal), post-trauma skin tears, cavity wounds, burns, and patients requiring education on wound healing and scar management.
There is an expectation to refer to the wider community team if required, support Senior clinicians and HCAs, and attend meetings when required. The post holder will help promote the service and implement changes as requested from the ICB.
Duties and Responsibilities
* Maintain good standards of communication in written and oral form.
* Assess wounds and select appropriate dressings for the care plan within local formulary and national guidelines.
* Prescribe safely within the prescribing competency framework and undertake prescribing audits relevant to the service.
* Assess patients autonomously within scope of practice.
* Demonstrate evidence-based practice in wound care.
* Promote and implement services to new sites as required by the ICB for service expansion.
* Document clinical interventions in patient records and the clinical notes using the service area computer programme ADASTRA.
* Complete associated documents for specimens using Cyberlab where available and label specimens.
* Work well within a small team and be flexible with working patterns across current locations to cover annual leave or sickness.
* Communicate with patients and carers in a polite and clear manner; relay information to and from other departments via telephone or email.
* Contribute to relevant audits to improve service provision; ensure mandatory training is up to date.
* Be aware of safeguarding issues and refer to local social services as appropriate (staff should be trained to level 3 safeguarding through mandatory training).
* Keep up to date with practice through educational events and guidelines (local and national).
* Awareness of communication barriers and adapt to needs (speech, hearing, language, emotion).
* Encourage patient adherence to programmes of care as required.
* Initiate emergency action when required; inform the Clinical Manager of untoward incidents and work within clinical governance framework.
* Dispose of clinical waste safely and contribute to infection control measures by adhering to policies/procedures.

Qualifications & Experience
* Active NMC registration
* Good standards of general education
* Experience in wound care
* Non-medical prescribing qualification
* Desirable: Mentorship qualification; completed the wound care alliance training
Essential / Desirable
* Desirable: Health promotion experience; understanding of current urgent and emergency care issues
