<div><p>The Unplanned Community Nursing Team reflects an integrated approach that has been designed in response to the changing needs of the local population, with a special emphasis on the proactive vision of care for patients in the community setting. 24/7 service that can react to their needs as and when required.</p><p>As the unplanned care team we cover the whole borough of Doncaster and provide all aspects of nursing care to patients in their own homes. We offer a supportive environment with opportunities for further career development, a strong in‑house training programme, links with partner universities and a comprehensive 12‑month preceptorship for all staff new to the post.</p><p>We believe in care that treats the whole person, not just the condition, enabling patients to stay at home longer and leave hospitals faster. Community nurses work closely with members of the wider multi‑disciplinary team, including GPs, local hospice, Clinical Lead Nurses and specialist nurses.</p><p>We are seeking to appoint a Staff Nurse who can demonstrate the vision and skills to work innovatively, flexibly and in partnership with other agencies, supporting the MDT in the assessment and provision of care to the high standard we aim for.</p><p>This role involves assessing, planning, implementing and evaluating nursing care in the patient’s own home and over the telephone. The post holder will utilize excellent communication, IT and clinical skills, telephonic clinical assessment, diplomacy and judgement to manage referrals into Community Nursing effectively and engage relevant services within appropriate time‑frames.</p><h3>Key Responsibilities</h3><ul><li>Setting, maintaining and delivering high standards of care in a variety of locations.</li><li>Under the supervision of a Senior Nurse, triage incoming referrals, assess the patients needs and determine the priority of care accordingly.</li><li>Adopt a multi‑system approach to case management, linking in with case managers and the wider health and social care community to ensure a seamless patient journey across the Doncaster area.</li><li>Provide first‑line management of junior staff within the team, offering support, mentorship and acting as a positive role model to maintain high standards and adherence to best practice evidence.</li></ul></div>
