Birmingham Community Healthcare NHS Foundation Trust (BCHC) is looking for Level 5 Bank Community Nurses to join our Early Intervention (EICT) and Community Teams within Birmingham.
EICT is part of a broader Early Intervention programme that has significantly reduced hospital bed days and saved £25.8 million for Birmingham, enabling reinvestment in health and social care.
You will be part of a large health and social care transformation, delivering high-quality care in patients' homes and community settings, helping to prevent hospital admissions and facilitate early discharges.
Requirements and Responsibilities
1. A full driving license and access to a car with business insurance are essential.
2. Work within an integrated service that builds relationships, reduces delays, and empowers staff to make decisions.
3. Provide nursing assessments and care planning, including referrals, in areas such as personal care, catheter care, wound management, and long-term conditions.
4. Operate as an autonomous practitioner within the community, conducting home visits or seeing patients in clinics.
