Patient Flow Facilitator

We are looking for an enthusiastic and hardworking individual to join this exciting new post within the Integrated Discharge Team. This dynamic team brings a variety of skills and knowledge. The successful candidate will be responsible for optimising hospital bed utilisation by managing the Ealing Bridging process and pathway. This will help to reduce the length of stay by ensuring timely, safe transfer/discharge of patients to the most appropriate and agreed environment.
Main duties of the job
Have the professional knowledge and leadership skills to take the lead in troubleshooting, communicating and assisting the Integrated Discharge Team (IDT), Ward Discharge Practitioners and Ward Managers/Matrons/MDT to ensure that all electronic communications relating to length of stay and discharge is updated daily.
Instigate and co-ordinate daily meetings with ward discharge co-practitioners/ nurse in charge/IDT to understand and act upon required treatment plan, escalations. To attend all routine and emergency bed meetings, and give feedback on escalations.
Work in close partnership with all hospital colleagues including all members of the multidisciplinary team across health and social care.
Operate independently and proactively to ensure safe and timely discharges.
Single point of communication to manage patients under the bridging pathway.

Education / Qualification
* Evidence of extensive experience and knowledge in complex discharge planning; ability to teach and assess learning needs of junior staff.
* Evidence of working across different agencies, recognising the contribution made by each agency.
* Educated to diploma level or relevant professional development.
* Working experience in both Health/Social Care setting.
* IT Certificate in advanced Excel/Access and Word.
Knowledge & Experience
* Experience of discharge planning within the acute setting.
* Experience of working with older people and their specific needs.
* Proven experience at working across directorate.
* Experience of managing complex situations and suggesting/discussing solutions which result in positive outcomes.
* Understanding of Discharge to Assess Pathways.
* Experience of working alongside community partners.
* Understanding of the principles of patient flow or similar operational role.
Skills
* Excellent organisational skills, communication and documentation skills.
* Excellent interpersonal skill and risk management.
* Well-developed negotiating and motivating skills.
* Able to deal confidently and professionally with all members of the MDT, internally and externally to the organisation, including patients and their families.
* Able to collate and present written, electronic and verbal information in a concise format from complex multi-factorial sources.
* Evidence of leadership skills and experience.
* Awareness of patient flow within an NHS environment.

Salary
£45,953 to £54,254 a year (Per annum inclusive HCAS pro rata).
