Job Title: Short Breaks Practitioner (CYPDS 0-25)Location: Kings Cross / Kentish TownContract Type: Full-time, PermanentSalary: Competitive
Our client is seeking a compassionate and organised Short Breaks Practitioner to support children and young people (0-25) with disabilities and complex needs. You will lead on assessments and reviews of social care packages, working closely with families, education, health and social care partners to deliver personalised short break support.
Key Duties:
* Manage a caseload of children and young people with complex needs
* Undertake strengths-based assessments and reviews
* Work with multi-agency professionals and contribute to EHCP reviews
* Support families to access community resources and navigate available services
* Align support with "Preparation for Adulthood" pathways from Year 9
Requirements:
* Experience working with disabled children and their families
* Knowledge of the Children Act, SEND Code, and relevant frameworks
* Strong communication, assessment and multi-agency working skills
* Enhanced DBS
