Senior Clinician - Older Adults Hospital at Home

The closing date is 02 July 2025

Hoursof operation: 8.00am 8.30pm, 7 days per week.

TheOlder Adult Mental Health Hospital at Home teamwork with patients during acuteepisodes of mental illness or mental disorder to provide an alternative topsychiatric hospital admission. We provide an early referral service forcommunity teams to support patients either in crisis or to prevent a further deteriorationof their mental health. OAMH Hospital at Home Team work closely with the OlderAdult inpatient wards to ensure they identify patients who can continue theirtreatment at home and facilitate an early discharge. The service aims toempower and enable patients and their carers to take an active role in theirtreatment.

Theservice aims to work together with patients, their families, colleagues, andthe wider community for outstanding care, enabling older people to live welland thrive at home.

Wewill also provide you with resources to develop and progress a long-term careeras well as flexibility, continual professional development as well as a widerange of wellbeing and support.

This is full time post, but we wouldconsider applicants looking for part time.


Main duties of the job

* We aim toprovide a response to all referrals within 24 hours to include early referralsfrom community teams
* We prevent admission by providing intensive support using evidencebased interventions whilst working collaboratively with patients and carers
* We ensure all potential admissions are considered for the OAMHHospital at Home service
* We facilitate early discharge planning with inpatient serviceswhich is recovery focused
* We are invited to attend the weekly rapid reviews for theinpatient ward
* We work with other professionals and services involved in thepatients care to provide a comprehensive service.
* There is high level of independent work as well as being part of ateam
* We cover the whole spectrum of mental health conditions and havethe aim to expand the service further by accepting referrals for patientsliving with dementia
* We promote the triangle of care to develop and formulate recoveryand crisis plans with patients and their families
* We complete offer and complete Carer Screens
* We will liaise with external agencies and Community Mental HealthTeams to ensure the best outcomes for the patient whilst supported by theservice
* We conductComplex assessments during the initial contact with a patient
* Wedelegate and manage the shift through an allocation and board round in themorning

Knowledge

* Physical Health knowledge
* Capacity assessments
* Socials Care systems and processes


Additional Criteria

* Share ideas around Quality improvement on patient care and systems.


Skills & Abilities

* Team player
* Enthusiastic learner and willingness to advance their knowledge and engage in practice development activities.
* Works well within a multi-disciplinary team, respecting all expertise and contributions.


Qualifications

* Relevant professional qualification relating to Health or Social Care e.g. in social work, nursing, OT with current professional registration
* RMN, BSc/DipSW, BSc OT
* Recent evidence of continuing professional development
* Practise assessor for students.
* Training or an interest in Solution Focus therapy and Cognitive Behaviour Therapy.
* Non-medical prescriber or an interest in becoming a non-medical prescriber.
* Preferably a degree, however diploma qualification also acceptable.


Experience

* Experience of working in a Mental Health setting
* Assessment of people presenting with mental health problems
* Assessment and management of clinical risk
* Working within a multi-disciplinary team
* Managing own case load
* Older Adult Mental Health experience.
* Work in the community.
* Experience of delivering evidence based psychosocial interventions.
* Acute pathway experience.
* Working with people living with dementia and their families.
* Undertaking initial assessments.
