About the Service
The Complex Older People's Service plays a pivotal role across the patient pathway, delivering:
* Acute frailty services at the front door, in close collaboration with the Emergency Department and Acute Medical Teams
* A range of specialist clinics supporting older people with complex medical needs
* Inpatient care across three consultant‑led teams on a dedicated ward
* Virtual wards supporting and managing patients in the community
The Role

You will be committed to delivering high‑quality, patient‑centred care and to supporting the ongoing development of innovative service models.
