The Cambridgeshire Learning Disability Partnership (LDP) provides an integrated health and social care service for adults with Learning Disabilities living in Cambridgeshire
The LDP aim is to ensure the assessment and provision of person-centred specialist health and social care services that promote an individual’s right to choice, independence, health and wellbeing, in line with their assessed needs and enables them to reach their full potential.
We have an exciting opportunity to recruit enthusiastic Band Community Learning Disability Nurses to work across Cambridgeshire
The posts are based in Cambridge South, the base is Papworth., Fenland Team, the base is March Cambridgeshire and East team, the base is in Ely
You will have experience relevant to working within a community-based role, supporting people with learning disabilities. As an autonomous practitioner you will manage a caseload providing assessment and interventions in relation to the health needs of adults with a learning disability.
Main duties of the job
The delivery of specialist nursing within the LDP
1. To participate in the delivery of specialist Learning Disability nursing service under the guidance of a senior Nurse.
2. With support from a senior Nurse work with identified service users providing assessment, identification and treatment of the health needs of adults with a learning disability.
