Key Responsibilities :
* Conduct home visits for patient assessments, therapy provision, and equipment provision, working independently or alongside a dedicated team.
* Collaborate with multidisciplinary teams (MDTs) to support urgent community responses and discharge processes.
* Provide comprehensive occupational therapy services, ensuring each patient receives personalised and attentive care tailored to their needs.
* Maintain accurate patient records, documenting progress and adjusting treatment plans as necessary to ensure optimal outcomes.
Requirements :
* A full driving licence and access to a car is essential due to the community-based nature of the role.
* HCPC registration is required to practice as an Occupational Therapist in the UK.
* A minimum of two years’ relevant experience, particularly in community-based settings.
* Experience working with MDTs, supporting urgent care responses, and contributing to discharge planning processes.
