Overview

We are looking for an experienced ACP/Specialist Paramedic/Nurse Practitioner for our Home Visiting Team and practice based clinics. The post holder will provide assessment, differential diagnosis and treatments of acute episodes of minor health and minor injury conditions alongside a home visiting service for appropriate patients. The role includes home visits, so access to a car and a clean driving license is required. The post holder will see patients autonomously, providing face-to-face appointments, home visits and telephone consultations for patients within the Practice and at patients' residences.

Main duties of the job

* Provide autonomous clinical care including face-to-face appointments, home visits and telephone consultations within the Practice and patients' residences, demonstrating critical thinking in clinical decision-making.
* Operate within your scope of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
* Prioritise and triage patient needs, making necessary referrals for investigations as appropriate.
* Manage patient caseloads for treatments, referrals, admissions and discharges as appropriate.
* Collaborate with the general practice team to meet patient needs, promoting self-care and following practice policies; support from the supervising doctor is available if needed.


Clinical Practice

* Assess, diagnose, plan and implement care for patients with undifferentiated presentations.
* Clinically examine and assess patient needs from physiological and psychological perspectives and plan care accordingly.
* Diagnose and manage both acute and chronic conditions, integrating drug- and non-drug-based treatments.
* Provide evidence-based practical skills in areas such as coughs, colds and flu; sore throats; earache; skin rashes; diarrhoea/vomiting; soft tissue and ligament injuries; wound care; abdominal pain; urinary/vaginal symptoms; sexual health assessments and investigations; emergency contraception.
* Prescribe and review medications appropriately within your knowledge and guidelines; prevent unnecessary hospital admissions by coordinating specialist care when needed.
* Support patients to adhere to prescribed treatments; provide information on medications and potential interactions; promote healthy lifestyles and self-care; assist with family planning or sexual health needs as appropriate.
* Liaise with patients, carers and colleagues to assess home environment safety and protect patients and staff from risk.
* Participate in audits/inspections and help monitor performance against clinical governance targets.


Qualifications

* Essential: Degree in a training programme approved by the relevant professional body; registration with HCPC or NMC; full UK driving licence.
* Desirable: Non-medical prescribing qualification; Master’s-level Clinical Practitioner capability.


Experience

* Experience managing a designated caseload, completing assessments, planning, implementing and evaluating care according to care plans; accountable for professional practice; ability to work independently and in partnership with patients and carers; capable of risk management and referrals as needed.
* Experience in primary care is desirable.

Other requirements

* Disclosure and Barring Service (DBS) check in line with safeguarding requirements.
* Current UK professional registration required.
