Overview
The Care Coordinator role at Libertatem Healthcare Group is a 12-month fixed-term contract opportunity in Wokingham. Main duties
* Develop and manage individualized care schedules, ensure continuity of care, liaise with stakeholders, supervise care and medication standards, and assist with operational tasks.
* Recruit and support the care team, foster collaboration, and promote ongoing training and development.
Responsibilities
* Care Coordination: develop tailored care schedules and ensure continuity during staff absences; maintain communications with clients and families; lead team meetings for collaboration and problem-solving.
* Staff Management: recruit and match support workers to clients; collaborate with HR and Clinical Leads to resolve staff issues; support ongoing training and development.
* Communication & Collaboration: liaise with internal and external stakeholders; supervise care and medication standards; maintain accurate records and report on client progress.
* Operational Efficiency: plan and deploy resources across care packages; assist with scheduling, billing, payroll, and on-call duties.
