We are looking for an enthusiastic and compassionate Registered Nurse to join our dedicated Frailty Unit on an eight-month fixed-term contract. This is an exciting opportunity to work in a specialist environment where you will play a vital role in supporting older patients, many of whom are living with complex health needs.
As a member of our multidisciplinary team, you will provide safe, effective, and person-centred nursing care that promotes dignity, independence, and quality of life. You will be responsible for undertaking comprehensive patient assessments, developing, and delivering individualised care plans, administering medications, and ensuring continuity of care through close collaboration with colleagues and families.
The Frailty Unit offers a fast-paced yet supportive environment where no two days are the same. You will have the opportunity to develop your skills in frailty care, working alongside experienced nurses, doctors, therapists, and support staff who are all committed to delivering the highest standards of care. While experience in elderly care or acute medicine would be an advantage, we welcome applications from nurses with a passion for making a difference and a willingness to learn.
Main duties of the job
To deliver a high standard of nursing care to patients undergoing surgical or medical treatment. The post holders will be responsible for the assessment of patient care needs, and implementation and evaluation of the programme of the care. The post holders are required to work as part of a team committed to delivering and improving a high standard of nursing care within South Warwickshire NHS Foundation Trust.

Job responsibilities

Clinical
Take clinical responsibility for patients within your allocated area of work
Continuously assess, plan, implement and evaluate treatment, using appropriate programmes/methods of care
To ensure a high delivery of quality care in accordance with Trust policies, procedures and guidelines. Participating in activities to improve the quality, productivity and effectiveness of care in response to local and national policies and initiatives
To practice in accordance with the NMC Code of Professional conduct and scope of professional practice and maintain own NMC registration
In the absence of a senior colleague, and following appropriate induction and guidance, will be expected to co-ordinate the shift, and take management responsibility for admissions, transfers and discharges to the ward or department.
Liaise with the multi-disciplinary team to ensure an effective flow and communications within the area, including being available for Board and Ward Rounds as required.

Qualifications
* Degree/Diploma in Nursing/Health studies
* NMC Registration
* Evidence of continuous professional development
* Non-medical prescriber
