This is an excellent opportunity for a Patient Services & Integrated Care Lead to join our diverse team of healthcare professionals dedicated to delivering patient-centred care that is efficient and impactful.

Our healthcare team significantly promotes health and well-being among prison populations, using strategies that instil hope for future resettlement into the community. We provide Mental Health, Physical Health, Pharmacy, and Clinical Substance Misuse services within an integrated healthcare model to enhance well-being and support better outcomes.

As a Patient Services & Integrated Care Lead, you will ensure high-quality, patient-centred care through effective coordination across prison and partner settings. Your responsibilities include overseeing the patient journey from arrival to transfer/release, focusing on complex releases, continuity of care, and safe transitions across all settings.

You will work closely with multidisciplinary teams, healthcare professionals, and community partners to manage patient engagement, address complaints, and contribute to service improvements. Maintaining accurate data and reports on patient flow, release planning, and engagement outcomes is also essential to inform operational decisions and ensure compliance with standards.

You will serve as a key liaison between clinical and non-clinical teams, advocating for a joined-up approach to care. Promoting health, educating patients, and engaging service users proactively are central to this role, supporting individual and public health objectives.

This highly collaborative role requires strong leadership, excellent communication skills, and a solid understanding of integrated care pathways and patient service operations.

Oxleas provides a wide range of NHS healthcare services in community and secure settings, including community health, mental health, learning disabilities, and secure hospital care. Our multidisciplinary teams serve people of all ages, working closely with NHS, local councils, and voluntary sectors across numerous locations in the South of England, including hospitals, clinics, prisons, and community settings.
