Job Summary
The main purpose of the job is to act independently to manage a caseload of patients referred to Discharge to Assess beds (D2A), and to provide full comprehensive assessment, diagnosis, treatment/appropriate management, onward referral and discharge to this complex range of patients. The supporting admin team will ensure the most up-to-date patient list is provided. The post holder will also manage a caseload in a primary care setting, dealing with a wide range of health needs and ensuring the highest standards of care for all registered and temporary patients.
Main duties of the job
Clinical Responsibilities: In accordance with the Practice timetable, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.

Job Description
Job responsibilities
* Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation.
* Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness.
* In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
* Providing counselling and health education.
* Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
* Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes and complying with QOF requirements.
* Delivering enhanced services.
* Compiling and issuing computer-generated acute and repeat prescriptions (avoid handwritten prescriptions whenever possible).
* Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
* Undertaking all the normal duties and responsibilities associated with a GP working within primary care.

* Experience of working in primary care in an area with significant health problems
* Experience of providing primary care in and out of hours
* Experience of audit and improving care for patients

* Full license holder with access to transport, as the post will require travelling to practices
