Raising the bar for people-centred outreach services, you'll enable people to learn the skills and knowledge they need to understand and manage their own situation more independently. No matter how complex someone's mental health needs, you'll provide support that helps individuals to decrease isolation, integrate into the community and make the most of every new opportunity.

The role involves rotational shifts including evenings, weekends and bank holidays. However, we are happy to discuss flexible working, promoting work/life balance.


Role Responsibilities

* You will be supporting clients in the community across Manchester, encouraging them to engage in drop-in activities in community centres, attend medical appointments, and engage in social inclusion activities.
* You will work with clients in their own homes, ensuring they are supported in taking medication, assisting them in activities of daily living, and provide emotional support if necessary, reporting any concerns to the Outreach Manager or the Nurse Manager.
* Working with qualified nurses as an Outreach Support Worker, you'll recognise indicators of deteriorating mental health and help to implement interventions.
* Promoting independence, well-being and healthy life choices, you'll also get involved in activities such as walks, theatre trips, gym sessions and building social networks for clients in the community.


The Ideal Candidate

* Experience of working with people with long-term mental health issues and a solid understanding of the recovery model.
* A great team player with lots of energy, strong communication skills, a flexible approach and a positive attitude to achieve targets in a challenging environment.
* With empathy, understanding and patience, you'll love talking to people and be passionate about helping them.
* Having a driving licence is highly desirable.
