Job Title: Advanced Clinical Practitioner - Urgent Community Response
Salary: Band 8a: £55,690 - £62,682
Hours: 27 hours per week
Contract: 12 months fixed term
Responsibilities
Our Urgent Community Response team, part of Locala’s Integrated Community Services, is comprised of Nurses, Occupational Therapists, Physiotherapists, Advanced Clinical Practitioners, Assistant Practitioners, and Healthcare Assistants. An opportunity has arisen for a motivated Advanced Clinical Practitioner to join our team.

The service covers community settings and offers a 7-day service, from 8am until 8pm, providing assessment, treatment, and evaluation of clients’ needs in a short-term intervention program. The team’s aim is to prevent unnecessary hospital admissions and provide urgent community response.
The post holder will require excellent communication skills to build complex relationships and network with key stakeholders to enable integration. Using initiative is essential, as is skill sharing with colleagues within the team and the wider organization. It is essential that all team members work closely together to truly integrate an MDT and provide a comprehensive service, utilizing the Calderdale framework.
