What you will do Undertake comprehensive assessments, reviews, and support planning in line with the Care Act 2014. Manage a complex caseload of adults with learning disabilities, delivering timely and effective interventions. Carry out Mental Capacity Act assessments and make Best Interest decisions when required. Lead safeguarding enquiries under Section 42 to protect vulnerable adults. Work collaboratively with health professionals, service providers, families, and carers to deliver holistic care. Support service users to access appropriate resources, promoting independence and improving quality of life. Prepare detailed reports, attend multidisciplinary meetings, and contribute to case conferences. Maintain accurate and up-to-date case records in line with statutory and local authority requirements. Contribute to service development and continuous improvement initiatives within the LD Community Team.
