The Older Adults Service works with older adults who have mental health needs in the community. The multidisciplinary team includes occupational therapists, nurses, psychiatrists, psychologists, social workers, voluntary sector and peer workers. The service collaborates closely with health, social care, statutory and voluntary partners.
The focus of the position is to maintain patients at home while supporting a strengths‑based approach to recovery. The role will be based in community teams, working within people’s homes to support admission prevention, quicker discharges and to support carers.
Key Responsibilities
As part of the Older Adults Hospital at Home service you will:
* Provide care, support and treatment to older adults with mental health needs in their own homes.
* Increase stability and quality in the lives of patients and their carers/families through timely, clinically effective health and social care interventions.
* Promote patient participation in their own recovery and help them develop coping strategies, taking gender, cultural and personal preferences into account.
* Use a strengths‑based risk assessment approach to promote positive risk‑taking.
* Support patients to engage with community‑based and neighbourhood services to help avoid unnecessary hospital admissions.
* Improve individuals’ social functioning and inclusion, tackling issues such as loneliness and isolation through collaboration with other agencies.
* Work in partnership with carers so that they are supported in their role and signposted to appropriate services.
* Protect the emotional and physical safety of staff in line with risk management, staff wellbeing and supervision policies.
* Partner with local authority social care colleagues to promote holistic assessment, care planning, interventions and outcomes.
The service has received an overall rating of Good from the CQC and Outstanding for care and compassion. It is aligned with the Fit for the Future 10‑Year Health Plan for England (2025) and the move from hospital to community provision.
Qualifications and Skills

* Can work confidently independently as well as part of a team.
* Have good communication and problem‑solving skills.
* Understand the needs of older adults and can demonstrate the Trust’s vision and values in practice.
* Are motivated to be involved in innovation and service development.

Staff will receive comprehensive support, including supervision, management from the Trust, and access to professional development opportunities to enhance skills and reach potential.
