Overview

The post holder will have responsibility for assisting in the early identification of people who require assessment of care needs to facilitate effective discharge from hospital. The post holder will ensure effective communication between discharge support services internal and external to Health, supporting high quality holistic discharge planning for people currently receiving care in hospital and their carers, particularly those who will require additional support from community-based services on discharge. The post holder will lead education of ward staff around the completion of referral information to a range of community-based services and will implement joint health/social work discharge planning standards, providing specialist advice, support and education to staff within the Hospital and multi-agency team on the discharge planning process. The role reports to the Integrated Service Manager and provides leadership related to discharge coordination, including the development and practice of staff within all areas of the hospital.


Responsibilities

* Assist in early identification of people who require assessment of care needs to facilitate discharge from hospital.
* Ensure effective communication between discharge support services internal and external to Health to support high-quality discharge planning.
* Lead education of ward staff around the completion of referral information to community-based services.
* Implement joint health/social work discharge planning standards and provide specialist advice, support and education to staff within the Hospital and multi-agency team.
* Provide leadership related to discharge co-ordination and staff development within the hospital.
* Liaise with ward managers regarding staff training needs to deliver good quality information to community-based services for effective discharge from acute care.
* Maintain knowledge of available community-based services to support people to live and die in their own homes and share this information with acute-care staff.
* Generate and maintain information on the status and numbers of people ready for discharge and liaise with Housing Social Work services, Hospital ward staff, Community and Rehabilitation Teams and Community Nursing services regarding discharge arrangements.

What we'll need you to bring

* Registered Nurse or AHP with valid NMC / HCPC Registration.
* Educated to/working towards/operating at Degree Level.
* Postgraduate expertise within speciality/area of practice demonstrating leadership and breadth of knowledge.
* Comprehensive knowledge of clinical guidelines and standards within the speciality/area of practice.
* Specialist knowledge and experience of relevant conditions, policies and procedures to deliver leadership within the area of practice.
* Management qualification or equivalent gained through experience and CPD.
* Evidence of Continuous Professional Development in area of speciality (study days/courses/postgraduate qualification).
* Excellent teamwork, leadership, motivation, and ability to work on own initiative.
* Excellent listening, communication and interpersonal skills.
* Effective time management skills.
* Working knowledge of basic information technology.

* 37 hours


Location and Working Pattern

Based in Frailty within University Hospital Wishaw.
