What the Job Involves The job involves conducting care management assessments to identify the needs and desired outcomes of service users and their carers. You will promote independence and recovery through enablement and community service linkage. This role includes collaborative work within multi-agency teams for seamless healthcare services and involves creative support planning with personal budgets. You will ensure adherence to risk management and safeguarding procedures, recognizing any children’s safeguarding concerns. You will manage case work, conduct assessments, and ensure correct recording of information. Supervising staff, promoting self-help initiatives, and engaging in partnership and team collaboration are key components. Furthermore, adherence to legislation, council policies, and codes of conduct is essential.
