To facilitate multidisciplinary patient care in the role of named nurse/team leader. Assess, plan, implement, and evaluate patient care. On a regular basis, accept responsibility for the management/leadership of the ward/department in the absence of the senior nurse. Participate in the supervision and teaching of staff and students.


Service Delivery Responsibilities

* Demonstrate up‑to‑date knowledge of national and local professional issues and lead/facilitate implementation at a local level.
* Undertake any other duties required by the line manager appropriate to the grade and relevant to the post.
* Act on behalf of patients, carers and staff to ensure their views are represented.
* Implement the local and national Clinical Governance agenda.

Clinical Responsibilities

* Support the ward/department Sister/Charge Nurse and their deputy in the implementation of their role.
* Practice as a named nurse/team leader taking responsibility for a defined caseload of patients on a continuing basis.
* Assess, plan, implement and evaluate patient care for a defined caseload.
* In the absence of Sister/Charge Nurse, effectively manage complaints and report to Matron.
* Promote collaborative working partnership and effective communication between all members of the multi‑disciplinary team.
* Ensure clinical incidents/near misses within the sphere of responsibility are managed in a timely manner. In the absence of the Sister/Charge Nurse report action taken to minimise risk.
* Develop links with all care agencies to facilitate seamless care delivery.
* Promote health and wellness through practice and education of patients, relatives, carers and staff.
* Take responsibility for developing innovative practice and leading the implementation of such practices, to enhance patient care.
* Maintain patient confidentiality at all times.
* Minimise hazards in the working area and report any identifiable hazards to the line manager. Adhere to all safe systems of work applicable to the work area.
* Undertake risk assessments and implement risk reducing measures. Report to the line manager any deficiencies in the arrangements for minimising risk.
* Promote awareness of child protection issues referring to area child protection policy and Trust policy. Attend child protection training in accordance with the policy.
* Promote awareness of adult protection issues referring to the Herefordshire Multi‑Agency Policy and Procedures for The Protection of Vulnerable Adults from Abuse. Attend adult protection training sessions in accordance with the policies and procedures.


Budget Management Responsibilities

* Actively participate in the effective and efficient use of the ward/departmental budget.


People Management Responsibilities

* Deputise in the absence of the Sister/Charge Nurse.
* Manage own time and that of others, through delegations to ensure high quality patient care.
* Act as preceptor/mentor/clinical supervisor for junior staff and students.
* Contribute to the setting of yearly personal and professional objectives for junior staff reflecting personal development needs in conjunction with the ward/department philosophy.
