Overview
We are a friendly and well‑established NHS GP practice based in Exmouth, Devon, committed to delivering high‑quality, patient‑centred care. We are seeking a motivated and enthusiastic Care Navigator to join our administrative team on a part‑time basis (27.5 hours per week across three days: Tuesday, Wednesday, and Friday).
The role requires working Tuesday and Friday from 8:00 am to 6:00 pm, and Wednesday from 8:00 am to 6:30 pm.
Imperial Medical Practice is busy providing primary care services to patients in Exmouth and the surrounding community. The practice operates a triage system to assess and direct patient requests to the appropriate clinician or service, and promotes digital tools such as online consultations and the NHS App to improve access.
Role Summary
As a key member of our front‑line team, the successful candidate will support patients to access the most appropriate care, acting as the first point of contact both face‑to‑face and by telephone. The position involves a range of administrative duties to support clinic operations and to maintain high standards of patient service.
Key Duties and Responsibilities
* Act as a first point of contact for patients, providing a professional and courteous reception service.
* Process patient requests for appointments, home visits, and telephone consultations.
* Use care navigation skills to signpost patients to the most suitable clinician or service, including Pharmacy First and other local services.
* Manage and use the practice appointments system effectively.
* Process online consultation requests (both clinical and administrative).
* Handle incoming telephone calls, take accurate messages, and pass on information promptly.
* Process incoming emails and respond where appropriate.
* Process repeat prescription requests in accordance with practice guidelines.
* Scan, code, and file clinical correspondence.
* Maintain accurate and up‑to‑date patient records through data entry and information processing.
* File, retrieve, and manage paperwork in line with practice procedures.
* Provide general administrative and clerical support, including word processing, photocopying, and document management.
* Support clinicians and the management team with administrative tasks as required.
* Act as a chaperone for clinicians when necessary.
* Support reception/front‑of‑house duties as required.
* Maintain a clean, tidy, and welcoming reception and waiting area, including notice boards and leaflet displays.
* Open and/or close the practice premises and maintain security in line with practice protocols.
* Manage outgoing mail, including franking.
* Support health & safety, equality & diversity, confidentiality, and quality improvement initiatives.

* Experience of Microsoft Office software.
* Experience of dealing with the public/patients.
* Experience of SystmOne.
* Experience of primary care.

* Flexibility to additional working hours/able to work at the desired times.
Knowledge and Skills
* Excellent telephone skills and ability to make appointments for patients efficiently.
* Excellent general communication skills & attention to detail.
* Ability to assist the organisation of reception team and prioritise workflow.
* Knowledge of medical terminology.
