* Salary: £26,000 - £27,000 per annum pro rata, including Outer London Weighting (OLW). Plus, a yearly staff retention bonus of up to £2106.
* Hours: 37.5 hours per week (Monday-Friday, 9am-5pm)
* Contract: 1 year
* Reports to: Complex Needs Housing Team Leader
* Main base: Letchworth or Ware Wellbeing Centre, Hertfordshire, with countywide travel
Role Purpose
The Community Outreach Worker provides advice, information, onward referral, and holistic support to clients experiencing mental ill health or needing mental wellbeing support. The role involves monitoring client safety and wellbeing, offering independent support, and embedding a recovery-oriented, trauma-informed approach.
About The Service
The Complex Needs Housing Service works as part of a multidisciplinary team with housing, drug and alcohol services, and other agencies to support individuals presenting as homeless with complex needs.
Objectives
* Improve mental wellbeing of clients
* Provide early access to help and effective pathways to services
* Support recovery and independence
* Reduce reliance on crisis and enforcement services
* Offer assertive outreach to clients in crisis or with complex needs
* Collaborate within a multidisciplinary team
