The Cardiac Rehabilitation service has an exciting vacancies for a full time Band 5 Nurses. Both are 12 month Fixed Term Contract posts. This post is a community role within a specialist Nursing Team.
Main duties of the job
Assisting the community cardiac rehab nurses with caseload management, in clinics telephone & home visit patient contact. Running exercise classes and seeing patients on the wards

The service provides cardiac rehabilitation across Bradford for patients post heart attack, CABG & Valve surgery .We see patients in various locations home visits, clinics & telephone contact
Full supervision & training will be given for this role
Person specification
Experience
* Post registration experience in cardiac, acute medicine or community nursing.
* Experience in working with vulnerable groups
* Heart failure care experience
Skills
* Car driver / owner with vehicle to use for work
* Ability to work autonomously
* Clinical assessment skills
* Ability to support more junior members of the team and students
* Must be computer literate
* Needs to have an awareness of quality issues, audit and evaluation
* Evidence of ongoing professional development
* Must be willing to undertake further training and education as required.

Knowledge
* Understanding of Information Governance and Confidentiality
Qualifications
* Registered Nurse, level 1
* Teaching qualification (ENB 998 or equivalent or able to work towards )
* Post registration cardiac nursing qualification (eg ENB 124, CHD diploma)

You must have appropriate UK professional registration.
