Approved Social Worker Older Adult Community Mental Health Team
Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Approved Social Worker within the Older Adult Community Mental Health Team to work Full Time based in Guernsey.
The salary for this permanent Approved Social Worker job is up to £73,085per annum.
* To promote and deliver a high standard of care based on best practice to maintain patient/service user safety and provide an excellent.
* To coordinate the assessment of patients’/service users’ with complex needs including individuals with unconfirmed diagnosis.
* To coordinate the development, implementation and evaluation of specialised programmes of care utilising evidence and research.
* To lead a team of staff/provide specialist advice in acute and chronic situations ensuring that the patients/services users’ needs are met along with their support network.
* To ensure patient/service user involvement in the planning, delivery and evaluation of care, respecting their beliefs and dignity.
* To utilise data and information from a range of sources to identify patient/service user condition to guide decision making.
* To be accountable for ensuring that safeguards are in place for patients’/service users’ and escalate any safety, safeguarding or risk concerns following the appropriate processes.
* To build and advocate a therapeutic relationship with patients’/service users’ and their support network, communicating with them in a way that respects their views, language, cultural differences and cognitive function.
* To use and promote a wide range of communication skills that can be adapted to meet the needs of complex and challenging situations
* To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards.
* To act as a professional role model for staff, advocating for patients and service users and promoting high standards of practice and strong professional values.
* To take responsibility for maintaining own, and that of others within the service, professional development and revalidation requirements, ensuring completion of training in accordance with the Statutory, Mandatory and Essential to Role Training Policy
* To be accountable for ensuring that safeguards are in place for patients’/service users’ and escalate any safety, safeguarding or risk concerns following the appropriate processes.
Requirements of this Approved Social Worker role:
* A social Work Qualification or equivalent.
* Understanding of relevant legislation.
* Registered or eligible for registration.
