Overview
North East Derbyshire (North) Adult Social Work Team are seeking to recruit experienced Social Worker(s) to join our friendly, community-based team of Social Care Practitioners and Social Workers. The team are highly supportive and operates on a largely agile, flexible basis across North East Derbyshire, utilising link worker and champion roles to carry out Care Act (2014) telephone and face-to-face assessments and reviews.
Responsibilities
* Evidence high levels of autonomy and accountability with timely recording of interventions to progress overall case management.
* Build positive, professional links with partner agencies such as Health, Police, Housing and Private, Voluntary Independent sectors.
* Identify signposting opportunities to support carers, maximise independence and individual wellbeing with a focus on person-centred, strength-based and cost effective outcomes.
* Respond to changing situations, triage and assess incoming work to support Adults and their carers requiring urgent response.
* Manage a complex caseload while meeting statutory duties under Care Act (2014) and local authority duties of care.
* Lead on Section 42 Safeguarding and chair multi-agency meetings (e.g., Multi-Agency Risk Management) and understand Mental Capacity Act assessments (including Best Interest meetings, COP-DOLs and court work) with support for skill development.
Qualifications
* Registered Social Work qualification (essential).
* ASYE preferred; sponsorship not offered; current staffing may prefer candidates who have completed ASYE.
