What you will do
Undertake assessments of need, including complex care and safeguarding assessments, for children and young people with disabilities.
Develop and implement child-centred care and support plans, ensuring services meet the holistic needs of the child and family.
Monitor, review, and adjust care packages, liaising with health, education, and voluntary services to ensure integrated support.
Safeguard children and young people, identifying risks, responding to concerns, and initiating appropriate interventions under statutory guidance.
Person Specification
Qualifications: Degree in Social Work (BA/MA or equivalent) with evidence of continued professional development relevant to children with disabilities.
Registration & Compliance: Registered with Social Work England; knowledge of relevant legislation and frameworks including Children Act 1989 and 2004, Children and Families Act 2014, and Working Together to Safeguard Children. Enhanced DBS clearance required.
