About the Role We are working on behalf of Hackney Council and we are seeking a committed and organised Reviewing Officer to join the Adults Commissioning Review Team. This role plays a vital part in ensuring that adults receiving care and support under the Care Act 2014 receive services that are appropriate, person-centred, and deliver value for money.
You will be responsible for reviewing care and support plans, engaging with service users and providers, and ensuring commissioned services meet agreed outcomes and quality standards. The role supports independence, wellbeing, and compliance with statutory and council requirements.
Key Responsibilities
Conduct scheduled reviews of care and support plans for adults receiving commissioned services
Engage with service users, carers, and providers to gather feedback and assess satisfaction with services
Monitor commissioned services to ensure they are delivered in line with agreed plans and quality standards
Identify and escalate changes in need to qualified social workers for reassessment where appropriate
Maintain accurate and timely records on the Council’s case management system
Ensure all reviews comply with the Care Act 2014, safeguarding principles, and Hackney Council procedures
Work collaboratively with social workers, providers, and commissioning officers to support service improvement
Provide information and insights to support commissioning decisions and performance monitoringAbout You Essential requirements:
Experience working in adult social care or a related field
Previous experience reviewing care and support plans
Strong communication and interpersonal skills, with the ability to engage sensitively with service users and carers
Ability to manage a caseload, prioritise work, and meet deadlines
Knowledge of the Care Act 2014 and principles of person-centred care
Understanding of commissioning processes and quality assurance
